Completion of the following seven steps will determine your ability to participate in practices and compete in an NJCAA sport. Incomplete or missing items may jeopardize your eligibility or that of your entire team. You MUST complete steps 1-6 before being cleared for practice.

- Puma Athletics Eligibility Form
- The completed form will automatically be sent to the PVCC Athletics Department
- Student-athletes must have their Maricopa MEID and password in order to access and complete the form
- If a student-athlete is under 18, their parent/guardian will have to sign the form electronically
- When the student-athlete completes the form, the form will then be sent to the parent/guardian's email to sign. When the parent/guardian signs the form, the completed form will then be sent directly to the PVCC Athletics Department

- Email completed affidavit to athletics@paradisevalley.edu
- Health History Form and Physical Examination Form, including an EKG
- Student-athletes will print out both forms to take to their Doctors appointment for their physical and EKG
- Email completed forms including EKG to athletics@paradisevalley.edu
- Health Insurance Card (front and back) – if you have insurance
- Email copy of front and back to athletics@paradisevalley.edu
- Government issued photo ID (e.g. drivers license, passport, etc.)
- Email copy to athletics@paradisevalley.edu
- Official High School Transcript - Required for all incoming student-athletes
- Must be mailed or sent via Parchment to the PVCC Athletics Office
- Emailed or faxed transcripts will NOT be accepted
- Prior College/University Transcripts - Includes ALL transfer and dual enrollment classes
- Must be mailed or sent via Parchment to the PVCC Admissions and Records, do not send to PVCC Athletics Department
- Transcripts from other MCCCD colleges do not need to be sent
